Tattoo Consent Form
Please take a moment to review the information below and fill out the state required consent form before your tattoo. NOTE: The technician shall not perform a body art procedure if the client fails to complete or sign this disclosure and authorization form, and may refuse any client at any time for any legal reason.
By submitting this consent form and engaging in the activity of being tattooed, I do hereby agree for myself, that neither Huni Buni Tattoo, nor any of the artists or other employees of the organization shall be held responsible or liable for any negligence, implied or otherwise, of personal injury, death, property damage, loss suffered or sustained by me in connection with or resulting from any or all tattooing activities engaged in by me. Further, I do hereby assume for myself, all risk whatsoever of personal injury, death, property damage, or loss suffered in connection to, arising out of, or resulting from any and all tattoo activities engaged in by me, and absolve and release Huni Buni Tattoo, its officers and members, their respective administrators, directors, agents, artists and other employees of the organization of all liability therein. Further, I hereby release, waive, discharge and agree for myself, my heirs, executors, administrators and assignees not to sue, arrest, attach or prosecute said Huni Buni Tattoo, nor any of its officers or members, their respective administrators, directors, agents, artists or other employees of the organization for or on the account of any such personal injury, death, property damage or loss. It being my express intent and purpose to bind myself, my heirs, executors, administrators and assignees from pursuing legal action against Huni Buni Tattoo, its officers, members and their respective administrators, directors, agents, artists and other employees of the organization.
